This is Italian national in fair health, whose health insurance is "in Italy". Being over 80, it is hard for her to get coverage out of country. I wonder if there's U.S. coverage that includes medical air evacuation in case of an emergency.

In addition to mbrkatz17's answer, I would say she should get a short-term major medical policy in the U.S. If something happens to her here, transportation would probably only happen after stabilizing her condition and coordinating the evacuation. During that time, if she is in the hospital, this coverage will ensure she can be cared for here and won't rack up enormous bills. Many times, if you don't have insurance that pays here, you would be shipped to a "low-rent" hospital where they send the poor.
In California, Blue Shield has a good short-term, major medical policy for a low premium.

No, if you want evacuation coverage to bring her back home, that's something you'll have to add to her medical policy in Italy. "Repatriation" coverage always goes on your home policy.
