If it is his regular doctor and they already have his insurance info all you have to do is call the receptionist and tell them that someone else will be bringing him. You may just have to sign a piece of paper stating that it is ok for the doctor to discuss your child's care with that person. If you are leaving your son with Grandparents for a couple of days you need to write a note stating that _________ or _________ (put Grandparent's name in) in your and your partner's absence, are legally able to sign for medical care in case of an emergmency. Make sure they have your child's Medical ID or insurance card and sign and date it. They are good for 6 months.

This letter gives a designated person the right to bring your child to the doctor. In general, you write a short letter with your information (name, address, phone), child's name and DOB.
Date and sign the letter. Simply state that Mary Doe has my permission to take my son, _________, to Dr. Smith on August 31, 2006.

I don't know a lot about the role of this man and his superior, but I would assume it is like a boss. Your boss has the right to read any of your email. Just like public places have the right to search you. Also, the superior may have demanded, but the man you were speaking with gave them up. Also, as I priest, you take a vow of celibacy (spelling?). If his superior suspected a non-platonic relationship going on, he has the right to know the facts because that could prevent this man from becoming a priest. His superior is most likely a mentor... so he is trying to do the right thing for this man... and probably make sure he is serious on becoming a priest.

You do not need a specific form, you just need to write a letter and be sure to sign it with your legal signature. some offices will give you a special medical release form if that is the format that they prefer

It is called a Medical Power of Attorney. You should be sure if you make on out that you make it a limited Power of Attorney with an expiration date. They should never be for longer the one year.

You can actually just write a statement saying that ..... I ---- give -----permission to take my son---- to the doctor. Then, you both sign it and have it notarized. My mother and sister actually did this. So,(at least in my state) they accept this. Call and ask your doctor.

You need a legal guardien paper from the hospital to let someones else take your child to the hospital, those are good for a year.
