You must make know that you are the leader of her pack. Be consistant in taking her out. Praise her when she does go outside and does her duty. Use the same words each time you command her or disipline her. Keep it simple, but always make her understand that you are her pack leader or else she will be yours!

put her in the crate with a pee pad and put her outside every 15-20 minutes I did that to mine and I had him trained in like 2 weeks of having him and he was only 3 months old.

unfortunately small breed dogs are often more difficult to house train then larger breed. I would try feeding her at the same time every day, and letting her out at the same time every day. Another very affective way of house training is using a kennel. Most dogs will not urinate where they sleep.

Moniter when your dog eats and take her out 15-30 minutes later. Do not let her loose in the house, but instead keep a short leash on her. Be sure not to let her out of your sight!

get a sponge and try to catch her pee or cut a portion from the pad where she peed. and then put the sponge or the portion that you cut in the middle of a pad, that way she could smell it and do her bussiness... it works on my chihuahua.:D
