Actually, I think the law says that you can use any name you want as your own, as long as it's not for fraud.
There's nothing to stop you from changing your name, no matter if you have a birth certificate or not. (by the way, you can just order a new copy of your certificate, it's actually pretty easy)
The thing is that your fake "name" won't be acknowledged by the government for official stuff.

No because your birth certificate will have to reflect the name change as well. It's a legal document.

You can legally change your name to anything the judge deems OK. I doubt a bc is necessary but it would no doubt hasten the change....especially if your given name is a burden. You would be able to prove immediately why you need/want a name change.
This is in the US as I don't know about other countries.

Yep. Simply start using another name. Oddly enough, it's legal. You can get a new I.D. with the name , a new Social Security card, etc.
