There is NOTHING on the passport application asking about your criminal record.

You can apply online or print out a PDF version and send it in. Here is the link. The application lik is in the left sidebar - "Application for Passport DS-11."

Normally yes as long as you're allowed to leave the country with a particular felony conviction. The bigger question is whether or not you will be allowed to enter the foreign country you wish to visit. They may have restrictions that won't admit you so check that out first by calling the Embassy before you even worry about trying to get a passport. Even if it's issued it may all be in vain!

Reasons for passport application denial:

Outstanding Felony/Criminal Warrant (not parking tickets or civil infractions)
Prior Felony Conviction that prohibits international travel
Not all felony convictions prohibit international travel.
Applicants needing additional information regarding a prior felony or who are unsure whether they are able to travel internationally should contact their probation office regarding probation terms.

You could try asking for a one-way passport that will get you out of the country. The passport people might welcome that.
