I don't work for Massachusetts, but I can tell you how it works in my jurisdiction (which is much larger than the entire state of Mass.' DPH).
The health inspectors here are required to have a Bachelor's degree from an accredited college, university, or educational institution, which shall have included 30 semester or 45 quarter units of basic sciences, including a one semester course or quarter unit equivalent, including lecture and laboratory, in Chemistry, Physics, Biological Science, and Microbiology as well as a semester or quarter course in calculus or algebra.
Once you meet the minimum requirements, you have to take an exam to demonstrate your knowledge of envirnomental health codes. You are eligible for promotion depending on your level of experience or additional credentials you may have (like a master's degree or REHS certification and so forth).
