Will my sister need a liver transplant?

She has Primary billiary cirrhosis (which is nothing to do with drinking!) She's on lots of medication but seems fine.

Not necessarily. I assume she is seeing a heptobillary specialist. If not, she should. Don't leave her in the care of a primary physician or internist. While he/she may be good, it's better if she is under the care of a specialist.
