Don't use lady fingers. You can substitute that for sugar free cake. That way it comes even closer to the original.
Just leave out the sugar.
You are aware that the liquor has the most sugar in it?

Don't even bother. Even if you substitute artificial sweetener, you still have the lady fingers and the liquor that turns into sugar. This is not a dessert a diabetic should consider. The fat content is amazing because you need heavy cream and mascarpone cheese to make it authentic. The recipes listed above, even if they are delicious, are not tirami su. (disregarding that one sicko answer)

NO SUGAR ADDED HERE!
INGREDIENTS:
1/2 cup brewed coffee, room temperature
1/4 cup amaretto liqueur
1 (8 ounce) package cream cheese, softened
3/4 cup heavy cream
2 tablespoons vanilla extract
20 ladyfingers
1 tablespoon unsweetened cocoa powder

DIRECTIONS:
In a medium bowl, combine coffee and amaretto. Divide cream cheese in half and stir coffee mixture into one half of cheese. Set aside.
In a medium bowl, whip cream with vanilla until stiff peaks form. Fold whipped cream into remaining cream cheese.
Line a 7x11 inch dish with parchment paper. Place 10 ladyfingers in the bottom of the dish. Spoon one-half of the coffee mixture over the cookies. Top with one-half of the whipped cream mixture. Repeat layers. Sprinkle with cocoa. Refrigerate one hour before serving.

Sugar-Free Almond Ladyfingers
Ingredients
SLICED ALMONDS, 3/4 cup
ALL-PURPOSE FLOUR, 4 tablespoons
EGG YOLKS, 3, whisked with Equal Spoonful sweetener (3 tablespoons)
EGG WHITES, 3, Beaten into a medium meringue
EQUAL SPOONFUL SWEETENER
MELTED BUTTER (optional)
ESPRESSO (or sugar-free hot cocoa for dipping
Directions
Preheat oven to 400F. Line sheet pan with waxed paper.
Process almonds in blender or processor until ground.
Mix in flour.
Mix yolks into 1/3 of meringue.
Sprinkle in 1/3 of the flour/almonds. Fold gently.
Fold in the rest of the meringue and flour/almonds in two more equal batches.
Spread the batter ½ inch thick across the waxed paper or pipe with pastry bag and large round tip into ladyfingers.
Sprinkle with Equal Spoonful sweetener.
Bake 8-10 min. The sweetener on top produces some smoke so don't be alarmed, your ladyfingers aren't burned.
Brush sheet cake or ladyfingers with melted butter if desired.
Slice sheet cake into ladyfinger strips.
Sugar-Free Mascarpone Cream Mousse
Ingredients
MASCARPONE, 8 ounces
EGG YOLKS, 3
EGG WHITES, 3, whipped into meringue with cream of tartar and a little equal Spoonful sweetener.
CREAM OF TARTAR
FRESH WHIPPING CREAM ,1/2 pint whipped with a little sweetener (not Cool Whip)
EQUAL SPOONFUL SWEETENER, 3 tablespoons
MARSALA WINE, 3 tablespoons
Directions
Boil 1/2 cup chopped raisins in 1/2 cup water. Reduce by half. Strain.
Whisk egg yolks with Equal Spoonful sweetener.
Whisk egg yolks into mascarpone.
Add marsala or raisin extract.
Fold in 2/3 of meringue. Set aside.
Dip ladyfingers or strips into espresso or sugar free hot cocoa prepared. Line serving dish as desired.
Pour/spread mascarpone cream mousse into lined dish.
Fold remaining meringue into whipped cream.
Spread on top of mascarpone cream mousse.
Sprinkle with cinnamon and cocoa powders.
Using two chopsticks or long round objects (pens, skewers, wooden spoon handles, etc.), make small spiral swirls into dusted surface starting from the outside and go all the way around, then move in closer and go around again until you reach the center.
Refrigerate 3 hours.
