I don't wish to scare you, but this sounds like the classic stemming behavior found in autism-spectrum disorders. (I have 2 children affected with this; they are both high-functioning, bright and fun kids, so this is NOT the end of the world.) The language problem is another tip-off. It may just be that he is a nervous child. Or perhaps he has not been socialized much and is just stressed when put into new situations. If it is really a concern for you, ask his pediatrician.
Does he see someone for his speech problems? Has his hearing been checked lately? If he has a high stress level at age five, perhaps you need to seek some professional help for him. Please don't be ashamed to ask for help. It's better to admit that perhaps you need help with your son now, than wait until it's to late. (PS. I am a licensed social worker in Ohio)

no i don't think its tics it sounds more like frustration of not being able to talk and understand properly ,my son was the same due to minor brain damage when he was born we only found out the extent of the damage when he went to nursery they get frustrated because they cant understand seek some professional help with him. when he learns to communicate more it should stop but get a speech therapist to help as i can be very frustrating for the parents as well i know
hope things get better as I'm sure they will good luckx

Don't worry I'm sure he is nervous, perhaps he is not confident as he doesn't speak well! See your health visitor and see what she thinks, or your Dr, and don't get stressed out, they always try to come up with a name or syndrome these days! Just let them do their job but don't hang on every word they say! I'm sure he will be fine, but may need a hearing test and speech therapy!

This is nothing to be worried about, you will find it is caused by nerves. many children and shy adults go through this.
Look at some of the stars being interviewed and see how they fidget.
When I was a child it was naughty to fidget and often a child would be smacked for doing so, but I am glad we are more enlightened now.
If you still have worries take him to the doctor and he will reassure you.

This is known as displacement activity. We all do it, just watch people as they walk in to a room full of strangers. Watch politicians as they mount a podium, they will often straighten their cuffs or check their tie, it's exactly the same thing.

I touch my face and pat my hair all day long. I often don't even realize I'm doing it, it's simply habit. There's nothing wrong with him.

It seems normal. Just keep watching him to see when and how often he does it. If it bothers you that much, talk to his pediatrician about it.
