Yes, the testes are removed, but the "sack" stays. The sack will usually shrink after a few months with nothing inside it.
It has nothing to do with a dog's dignity. If your dog's dignity only resides in their genitals, that's one sad dog! My dogs' dignity resides in their fine temperments and excellent obedience to me.

I'm not quite sure what your asking. Yes, when a male dog in neutered his testicles are removed. This prevents reproduction and also, decreases behavior probloms & aggresiveness. I would totally suggest someone having this procedure done on their pet. If your dog could, he would thank you very much. It's not painful at all.

Yes, but you can ask the vet to put plastic "balls" into the excess skin if you want your dog to look anatomically correct, but not be able to reproduce. Not all vets will do this though.
I've had this done to one of my males, when he had cancer in that area, they took the cancerous one out, and put in the plastic one, he can still reproduce, but looking at him a stranger couldn't tell which one he had cancer in.
