Well, in the letter you can state how happy/proud you are to receive such recognition. What does your company do? What is your job? Can you tie these things into the mission of the nonprofit?
For example: I enjoy working with the public. One of the favorite parts of my job is working on project x for your organization. These tie in well with my company's overall mission.
The plaque will look very nice next to my desk. This will remind me of my responsibilities to the company and to the public. The check is a wonderful gift. I plan to use it for x.
As for the ending:
Your company is generous (or words like that) and kind for recognizing me as Employee of the Month. I hope to continue to live up to your standards and expectations.

1) you must set up a criteria with its score for the recognition of the employee of the month
2) you must decide the qualification of employees who have the right to be selected
3)you must set up an adhoc committee for selecting it and the data which should be collected
4)you must give a certain period that should be accomplished by the committee
5)you should review the committee's report after they finished their assignment and decided the employee of the month
6)you prepared a check and a plaque for the selected employee and decided the date of ceremony
7)you must invite the related persons to the ceremony of the forwarding of check and plaque
8)you must prepare for a speech and maybe the snacks and drinks for it.

There are a number of good etiquette books but in lieu of that I would suggest that it reads something like this:
Dear (make sure you have someone's name),
I wanted to take this opportunity to thank you for the recognition your organization has given me in naming me your employee of the month.
In this busy world of ours it seems that saying "Thank you" to the people around us for their hard work and contributions is completely forgotten about.
So many people work so hard and give so much of themselves every day and it is such a blessing to know that there are still people out there who care and notice when someone does a good job for them.
I will proudly display this wonderful plaque you have given me and I am looking forward to getting myself (or someone else like a co-worker, spouse or child) a treat with the check you were so very kind in giving me.
Thank you again for all you have done and for reminding me how much it matters to feel appreciated.
I will definitely take a lesson from this and make sure to let the people in my life, who are a blessing to me, know how much I appreciate them.

Note: If you have the time and inclination you may wish to offer to volunteer some time to this non-profit organization.

I would follow the KISS rule (Keep It Short and Simple). Just let them know you received the award and the check and that you were both honored and grateful. In today's business world where time is precious, they will probably appreciate the brevity...and by keeping it concise, you do appear professional, as long as your grammar, punctuation and spelling are all correct. Less is more. I'd also send a card as opposed to a letter.

But, I can't tell you what to say, and this should come from "YOUR" heart, not mine.
But I can offer you some suggestions:
Keep it simple.
Recognitions are an honor, so mention that you feel honored.
Explain why you love your job?
How did the recieving this plaque and check make you feel?
Is there someone at your job who has contributed to your success, mention them briefly.
Lastly, remember to K.I.S.S. (Keep it Simple Sista)
A simple note of thanks, with a few lines from the heart, goes a long way.
