Go to the post office or county clerk and ask for a passport application. You can also download the form. Passports are good for ten years. If you are under 18, your parents have to sign. You will have to provide two passport size photos and send money order or cashier's check with your application. It runs between $60 and $97, depending on whethr you want it rushed.

Got to the main post office in your county you should be able to order it there.The cost is about 85 dollars

Post offices have forms available. You can oick them up and fill them in your own time. You'll need quite a few documents/cards to prove your identity.
Once the form is filled you can hand it in, and pay.......I can't remember how much it cost. But it was quite a bit.
