Yes. One from the router to the cable or DSL modem and one to your PC.

Unless it is wireless you need one to go from your modem and another from each computer that you plug into the router. If it is a wireless router you just need the one cable from the modem.

In most cases..yes. One to connect from the wall jack to the cable or DSL port, and one cable to connect from your computer to one of the ethernet ports.

Probably you can use USB as well, but it is more recommended to use Ethernet.
