Most cases of cauliflower ear are due to infection or infestation. Bacterial or yeast infection or ear mite infestation. Some animals just get it, for no apparent reason. Generally speaking, it's not fatal or harmful, unless it's certain bacterial infections. There are surgeries that can be performed to fix the ear, and to make it look more normal, but they can be pricey, and are illegal in some states, as it can be considered cosmetic surgery. Not every state allows for cosmetic procedures anymore. If the ear folds are red, thickened, or smell bad, then it needs to get to a vet asap for bacterial infection.

The problem is that the cat either has mites or an infection and scratched and shook it's head and a hematoma formed. It is a pocket of blood that forms between the layers of the ear. You friend didn't have it taken care of surgically, so the ear became full of scar tissue and shriveled.
The problems this causes is limited to being predisposed to more ear infections since there is limited air flow to the canal. Clean the ears out with Q-tips and mineral oil weekly and aviod a future problem.
Once there is a shriveled ear, it cannot be corrected.

The misshapen ears may be due to "aural hematomas". When the cats fight they can damage blood vessles in the ear flap which causes the ear flap to swell up like a puff pastry. As the swelling abates, the ear shrivels into an odd shape.
The problems this can cause is that if the malformation is bad, it can disrupt the cat's hearing, or cause a build up of wax (and/or yeast, bacteria) in the ear canals which can lead to infection and hearing loss.
Have your freind get his cat to its veterinarian to determine the cause of the malformations, and to find out if its causing the cat any problems.

Scottish Fold cats are born with those kinds of ears. It is the breed characteristic.
Something is going on with the cat and I feel it needs to be investigated right away by a vet.
