Just be cordial and keep things professional and to the point. That’s what I do with my boss. If you want to keep your job, you must smile, be nice, and do your work to the best of your abilities. The contact between my boss and me is minimal because I behave like this.

Difficult to say. If you believe your job is in question then I suggest you begin to document yourself by creating a diary.
Sometimes the worker/boss relationship is not meant to be and if you've been at your job a short period of type you should consider a change of your job.
If indeed you want to work things out you may considering taking the issues straight on and saying " For some reason I feel that we never got off on the right foot and I was wondering if you felt the same way" Then express what your goals are and what you would like best to make things work out.
If she is professional she'll try and work with you.

Do you want to stand out? Treat your boss like a liege lord and do what she or he asks you to do — cheerfully, without question, and to the best of your ability — and you will stand out. In your boss’s mind, you will contrast sharply with the people who don’t want to be told what to do. And it’ll be more enjoyable for you to be at work.

I did not get along with my boss because he lied to me several times and I hate liars so I got him sacked. I achieved this by writing a letter of complaint to the company concerned exposing the incompetence that everyone complained about but no one else had the guts to expose.

Sometimes if someone at work makes you miserable it is better to just leave. You can fight, or put up...but neither is worth the hassle if you can get a just as good or better job somewhere else.
