By "twins" do you mean his nipples or his balls?
In my experience, I have had guys that enjoy nipples stimulation, and some that really hate it. My husband hates it, so I dont even go there. Its a personal thing, I think.
As for the scrotum, almost every single guy I have had sexual contact with or even have talked to about sex or masturbation has mentioned that they like their sac stroked or sucked on or something like that. My husband has a running "joke" that masturbation is called "tickling your sac". Other guys, I know have said they stroke their sac while masturbating too, so I know he's not the only one.

That is called a "Twinkle Toe!" All guys aren't the same but I'll say I love it. You have to know what you're doing though or you'll get kicked in your face. And you can't do it for long periods, do short ones, do something else then go back. But all in all, go for it!

I dont think you mouth should be anywhere near the twins, unless he shaved them. Thats like porno style. Just worked the shaft, doing it while he is driving is really nice too.
