this guy is standing at the urinal next to me. suddenly he lets out a big gasp and grabs the urinal plumbing like he's gonna rip it off. I usually would help out a person (medically) but I didn't feel right talking to him with his willy out.

Maybe he has a bladder infection or STD? I wouldn't worry about asking him anything though. Pain is a strong alert mechanism and it's pretty clear he knows he has a problem.
Now if he couldn't walk or showed other signs he needed help that he couldn't fetch for himself (with his willy properly stashed first), then maybe you had a moral obligation to offer assistance.
If he'd had a heart attack, he'd have hit the floor.

You made a good decision. if he has an infection, or some medical condition thats on him. doctors choose to be doctors and talk to people about their urination issues. thats not your job.

As long as he didn't stay that way a long time I'd say you were better to leave it alone... If he got done and then still seemed really bad off I'd say help... Peace!

if he asked for help and didnt help him... then that would have been messed up... but ive gotten pains before in public where i cant hide my emotions but i dont need anyone to help... i just need to let it pass or what ever...
