First have the confidence that you can speak normally
Second, do tongue exercises - twisting and protruding - as many times as possible - preferably when you are alone - do this till you feel pain in the tongue - this should enable you to twist your tongue (not literally!)
Third, try to keep the naturally rounded stones available on river beds in the mouth while talking. This should control your stammering
Fourth, normally there is a tendency for the people with stammering to talk fast. If one can control the speech this stammering can be overcome.
Fifth, such persons find it difficult to come out only with a few words. They can identify such words and look for other easy equivalent words
Finally, if one has faith, s/he can recite the Sloka on Mookambika on a daily basis. This has proven to be a best medicine for stammering
You can practice any/all of these only if you have the confidence to speak normally

I saw a special on this very subject some time ago. There were (I think 6 ) people who were afflicted with this condition and they were each fitted with a rubber belt around the chest and they were all able to speak normally as a result of this. I don't know what they called the treatment but this gave them the confidence to push ahead with therapy to overcome the condition. Have you contacted a speech therapist? he might know how this treatment works but I saw it on the special and the look of relief for the participants as I know what a struggle it is for you as I have a brother who is afflicted with the same condition. Let me know how you went as someone must know about this type of treatment which, whilst it does not cure the condition, provides for freedom from the condition whilst the belt is in place. Old people says that by keeping pebbles in mouth n shouting tho the extent of your voice on sea shores in earling mornings giv rid to this problem. May be some psychological problem, if so consult psychologist or psychatrist.

My uncle stammered and he talked to a therapist who asked him to sing. When he sung he actually didn't stammer. (He wasn't a good singer or anything). I think it was something he practiced because he relaxed that way and wasn't self-conscious . He suffered from depression due to this issue but this technique helped him a lot.

sit in front of a mirror & imagine u r talking to somebody , don't get nervous. do these exercise daily. don't think it as funny.
actually stammering happens mainly due to nervousness, which can be avoided by speaking to people but not by avoiding
