Yes... It was quite some time ago and I was working in a large corporation located in Mexico. I worked as a purchasing manager and interacted with accounting department quite often. There was a heavy set woman assigned to purchase orders and for the most part we exchanged documents and conversations sitting down. The building had two floors with a staircase in a spiral shape and as I was climbing up the stairs I saw the woman coming down the stairs, but ever so gently, one step at a time. I politely asked if she hurt her leg and she replied "NO, I AM PREGNANT", and I felt every drop of blood in my body rush up to my head... For about a month or two, I was on constant business trips and I was glad to be back in the office and we happen to run into each other on the stairway. Of course I saw this as an opportunity to redeem myself for the last embarrassing situation and with a smile and a genuine gesture of concern I asked "when is the baby due?", she looked at me and I thought I saw a smirk and replied "My baby was born without complications, thank you".

Unfortunately yes I have. I was in the mall and saw a large woman in a store looking at newborn baby items. I struck up a conversation with her and then said " oh and congratulations, when is the baby due"? OH MY GOSH.....she looked at me with the strangest look on her face as she informed me that she was NOT pregnant, and that she was shopping for a shower gift for a friend.
I was totally embarrassed and just wanted to sneak out of the store unseen.......lol
I did however apologize to the woman before I left.

No I've never asked but one time a young child (about 5 yrs. old) asked me if I was pregnant. I wasn't so I told the child, "No I'm just fat".

No, but people have asked me. I feel for you, but you should never, ever ask a woman that unless you are sure. I've had two surgeries on my stomache, and it pooches a little. When people ask me that it makes me feel like an Elephant.

No, but on the phone I've said "sir" when it was a female caller, or the other way around. Sometimes you can't tell by a person's voice, or even his or her name. It's still embarrassing, though.
