make sure she likes you. you can usually tell. if your questioning yourself about it maybe you shouldnt ask her out. have confidence too. try flirting with her and being sweet. also look her in the eye and smile. that works GREATT. well when you know without doubt that she likes you, take her out somewhere and tell her your feelings straight out. maybe then shell come out and tell her feelings too. just take a risk and ask her.

well, it all depends. what happened with me is i found out a friend of mine had cancer, and i cried all over his shoulder for nearly an hour.... practically the whole school asked us if we were going out, and we decided to talk about it later, decided deciding was too final, and decided not to decide. four hours later our mutual friend was like "you guys should go out" and he turned to me with this big grin and asked "will you go out with me"
its really simple. be upfront about it.

well if you really like her please put some effort into it...maybe take her out to dinner or maybe invite her over to your place for dinner. after dinner casually talk and work your way into it. tell her how you feel about her and that should win her over.
