Probably should just buy a new bottle but if you are particularly attached to this one use some bleach and a bottle brush to scrub the mold away and then rinse it out VERY THOROUGHLY with hot water and let it air dry for a day or so before you use it.

a bleach water solution should do the trick, and go to the store and buy a baby bottle brush so you can clean it out properly from now on. When you're not using it, rinse it out and let that puppy air dry. that should help. Or put it in the freezer (I don't know what "nalgene" is, but if it's expensive, you may not want to do this because it may crack it.)

Fill it about half with water, add a little dish detergent, and a teaspoon of bleach & shake it vigourously & leave it over night. If there is still odor in the bottle put some lemon juice in it overnight.

Vinegar. (works good on bathroom gunk, too....like the stuff that gets on the showerhead and in between the faucet and the tile)

you can use diluted bleach, just make sure you rinse it well after wards. If this doesn't work, throw it away and get a new one.
