There are a few different ways to take that phrase.

(1) It could be the literal meaning of two hands being together. Such as a two people holding hands.

(2) It could be two or more items/topics/people that are used/work together in order to accomplish a common goal. For example: They worked hand-in-hand to rebuild the house torn apart in the storm.

According to the American Heritage Dictionary, it means "Cooperatively; jointly."

I think that most people would think of it as referring to two people holding hands, such as lovers walking down a path holding hands, and in that sense, I think the words 'cooperatively and jointly' apply. But it can, I think, also have a business conotation. You know, they were crooked business people, plotting their evil schemes hand in hand.

Were you listening to "were all in this together" from High SChool musical or something. Becuase thats part of the chorus. I would say it means holding hands and being there for someone
