I don't really know your whole situation but maybe your kid doesn't need a doctor. Doctors have a tendency to over diagnose and over medicat children without ever giving them a chance. It may be possible that your kid is just different then other kids. I would first of all sit down with your child and talk to him/her about what is going on. How do they feel inside and what exactly is bothering them. Also, you may think I'm nuts but check out the site below, maybe your answer is there.

Why do you assume your child needs a doctor?
Do you want to turn him into a drug addict? If you take him to a doctor that's what he will do! Ritalin, Aderal, that's all they know.
I would do a bit more homework, perhaps get a few good books on early child development before I went dragging my kid off to a doctor who's just going to make a snap judgment and medicate him anyway.

I would take your child to a counseling center, They all have people who deal with problems of only kids and they have different ways to find out what is really going on. Just make sure you do research on anything the tell you so you become informed and are not just going on word of mouth

Your child is probably normal, they are not minature adults.
They go through this stage, just have patience, and keep reminding your child . Other kids, wow, they have the same problems too with their parents.
Be patient, this all fades by time they are in 10th grade.

Ask for a referral from your family physician. You child could have ADDH, a socializasion disorder, or any number of things. Also they could be perfectly normal and just like to be alone. The part about following directions sounds like an attention deficit disorder. But see you child's physician or a pediatric doctor for a referral.

Your Doctor and your Insurance Carrier need to work together to have the child referred for Psychiatric/Psychological Testing to determine if there is a problem present.

I would get a referral from your family doctor or pediatrician. They would have the best idea of the type of doctor your child should see.
