the one in front of me

amount of contract
date recieved
termination date

a statement of parties involved, dated timeframe, all information of parties like addresses

"Riders" of work to be performed outlined and specified

a rider of payment provision, stuff like "not to exceed" $amount

and a place for the people in charge to sign and date there names.

then as an addendum go into greater details of all the riders detailing stuff like expected hours and by whom the work will be done
invoices and payments, that all reccords need to be immediately and readily be available for review
provisions saying that the work will be done by the specified consultant unless otherwise agreed ""no farming it out"
names and addresses of representatives for all correspondence to be mailed
provisions for extra costs
something saying that you are an EOE
a statement that the consultant wont work with other parties of competition or other, like a DOT
a statement saying that you can terminate the contract whenever you feel like it, if you ever feel like youre loseing out onthe contract
warranty
access to records
the consultant is to be familiar with all laws and ordinances relevant to and concerning the project

and whatever else it takes to CYA
