after the visa is issued based on relative (wife) how soon after all the final steps are completed and the visa is issued would she have to leave her country for the USA, i have been told that she needs to leave her country within 6 months,or it will be null and void, is this right???????

Every country is exactly the same; it's US law that says how long a US visa is valid for.
A US immigrant visa must be used within six months. If she doesnt use it but still wants to go, the visa must be replaced. That's not a simple process, since a lot of the documents have gotten stale and will have to be renewed. But she hasn't missed her opportunity in immigrate.

Each country is different. Look up embassy for that country and contact it.
