Try Inkscape - http://www.inkscape.org/ - it's free (and very Mac compatible). Just import your picture and away you go... It's similar to Illustrator but not expensive (obviously given it's free.) And if it's just simple text you want and lines, this will be better than photo-manipulation software as it's vector based, giving clearer, sharper resolutions.

open a drawing document in Appleworks. Drag the photo onto that. Resize as needed. Draw lines with the drawing tools. Open a text box using that tool, position where required and write text in there.
Don't listen to those other guys. This is Mac we're talking about.
