You need to write a letter to :
Central Intelligence Agency
c/o MkUltra
Office of Public Affairs
Washington, D.C. 20505
Request - Form D0.N0t.Pr013E.mY.M1nd
Upon receipt of said form, fill it out in triplicate; one for your own copy, one for the CIA's copy, and one for the Directorate of Science and Technology.
