You can either buy iron-on transfer paper and print your photo on it, as suggested in the previous answers, or you can buy freezer paper and follow the instructions in this tutorial:
http://limelady.livejournal.com/106122.html
If you use freezer paper, you'll have your image printed directly on the fabric.
If you use iron-on transfers, make sure you buy a kind that goes with all background colors if your fabric isn't white. Most of them are made for printing on white fabric.

Buy iron on transfer paper and print it from a computer. Then just iron it on. If you dont want to do it yourself any office supply place like staple or office max) can do the printing part of it. Be sure that if the photo has writing or anything that needs to face the right direction, to mirror image it first so that when you iron it on it will come out right.

There is iron-on paper that you can buy at staples or...wal mart maybe. Basically its a special paper that you can print pictures or text or whatever on and then you iron it on your shirt. Make sure that when you print the picture out its backwards or opposite the way you want it to look on the cloth, because when you iron it on it will come out the right way.

Put the image on to your computer. Go to your local business supply store and get iron on transfers. They don't hold up great after numerous washes but its great for little things.
