It depends on State, not local laws. Ohio just stated requiring ID's, but not photo ID's. Personaly, I think a photo ID should be required.

Not in California. In fact in California, it is illegal to ask for a photo id in order to vote.

I never had to show any ID at the polls. Your registered to vote at a certain poll in your hometown. They have your information if you sign the card wrong they may question you.

ceoends on where you live. In canada you only need your SIN and 2 other forms of proof of address. I took in my Sin, phone and hydro bill, birth cirtificate because I moved to another province, and my health card from the other province.

Don't make a fake ID (if you were thinking about it). If you do make a fake ID, you will get in far more trouble than thinking about it.
