All the time. My dog has a habit of sleeping right where I step off my bed, so first thing in the morning, I roll over and step on something furry.

Oh yeah. My cat was sleeping in the doorway once and I tried to step over her. Of course, that was the very moment she decided she wanted to get up and run for the kitchen.

Yes :-( usually a tail. We have 10 and they are literally underfoot, especially at "treat" time during the evening.
The worst one though was when my husband got out of the shower and a kitten ran into his foot mid-step. Knocked the kitten unconscious! We rushed it to the vet. It survived, but it was a horrible feeling.

An overweight friend of mine accidentally SAT on his cat when he was a kitten...
Hence, the name "Nermal" (as in "not quite normal") He'll yowl at you for no apparent reason...(he's even been to the vet for neurological screens)
He's 10 years old and (otherwise) doing fine...

My male cat is in the habit of sneaking up behind me while I'm working in the kitchen, and sitting down....I turn around and BAM! there he is....if I step on him at all, he lets out an unpleasant meow and runs away. I usually just laugh at him....because I swear he's doing it on purpose.

Yes. That is why I take my shoes off as soon as I get in the house. I can feel them better and avoid it. When I wear shoes, I react to their scream not noticing that I am actually stepping on them.

Do I ever? I own a cat that has miserable timing...he literally runs into my feet. Don't even get me started on him jumping - he went to jump on a window ledge once...did I mention that the window was shut? Ouch!

Absolutely. We all do. Just check to make certain they aren't really injured (which 99% of the time they are not), and reassure them that it's all OK. They are fine in a few minutes.
