Osteo arthtitis is the biggest, but others can include nerve damage resulting in loss of fine motor abilities and loss of feeling, and carpel tunnel syndrom.
Working on your hands doesn't only work on your hands, because the shock is transferred up your arms, so elbow, sholder, back, and neck problems can result as well.
Train carefully with proper technique and posture, working slowly to increase your strenght and tolerance. As always, check with your doctor before you begin any workout program.

Done correctly (like slapping your palm and backhand on a bag filled with your fave beads, rocks, BBs), there should be minimal effects. Done by karate freaks that like to break rocks with bare hands, yes, you will guarantee useless hands in the future. There are those who break the bones in their hands so they heal with calcification. They really end up with more brittle bones and dramatically hasten their impending arthritis.
The reason for hand conditioning is to desensitize your nerves. There is no self-defense application for brick or rock breaking.

The biggest problem we have is infections. People seems to think the goal is to hit until you bleed. That is nonsense.
If you continue to hit with an open cut you are causing damage and getting your nasty blood all over the makiwara.
Once you open something up, stop, clean it out and bandage it properly. Do not work that surface until it heals.
