Before the advent of modern medicine, giving birth was one of the most dangerous things that could happen to a woman. The mortality rate was ridiculously high. It's much safer these days, but misfortune still happens.

Absolutely. There are plenty of complications that can occur during childbirth where the mother can die. She could have excessive bleeding, for example, or catch a deadly infection. Anymore, it's rare that mothers die in childbirth because doctors can generally fix whatever happens to them in the course of delivery, but it's possible nonetheless.

yes and this is the reason why women must go to doc. when there pregnant, for the baby and themself. The more attention from a doc. you have the lower the chance of dying while giving childbirth.
Usually people that use midwifes die

Yes - women can die. Although it is a natural experience, there is a risk of great complications. That is why constant and good prenatal care is so important - to pick up on medical conditions that would lead to a higher risk of complications.

Yes, it can happen. It is still very common in developing nations (a major problem in Africa), but reasonably rare in developed countries. In the US, chances are less than 1 in 1000 ( I think it's about 1 in 1400).
In underdeveloped nations there is, unfortunately, a skew of things that can cause death (bad hygiene, no operating rooms, malnutrition, etc). In developed countries, it is mostly because of preexisting conditions (hemophilia, high unmonitored blood pressure leading to cardiac arrest, sometimes existing cancer of the cervix, aneurysm). It is very rare, but I know of 2 women who died because of an aneurysm- one in her 8th month of pregnancy, one during childbirth.
Still, the risk is minuscule, if the pregnancy is properly monitored.

Women can still die in childbirth but the risk is very low. The maternal mortality rate in the US is less than 0.01%
