I don't believe either of the first two answers quite address your question. You probably had a nail fungus. While nail avulsion is good, if it does not get it all, it can return. The fastest way to remove it is to get the nail avulsed again, then get either a prescription anti-fungal, or any strong anti-fungal herb. The book I am currently reading on yeast suggests tea tree oil or oil of oregano. It may take about 6 months to cure, as basically, you wait until the nail grows out. For the thickening, my podiatrist recommended "Nonex", however, I use this alone - as the nail avulsion apparently got rid of the fungus. I do not know if "Nonex" can be used with the tea tree or oregano oils.

Also, you may want to start eating plain yogurt about 3 times a week. The acidophilus bacteria in the yogurt neutralizes yeast.

Nail fungus is the same fungus that results in jock itch, athlete's foot, thrush (white tongue), vaginitis (in women), ringworm and other skin rashes. It is the Candida Albicans fungus (yeast).

Ouch, it is possible that you have an infection going on there, unfortunately it takes so long for an injured nail to grow completely out you may want to visit a doctor.

The nail is dead from being torn loose from blood supply. It should grow out a fresh nail.
