Ask your doctor what the procedure is. Some automatically always cut a first time mom. Others wait to see what happens. I had an episiotomy with my first child. My second was bigger, but my midwife carefully stretched me around his head as he came out, so no tearing or cutting. See if you can get the doctor to try stretching and slowly moving your skin for the head to pass before he just decides to cut.

Get a midwife. It is a very unusual doctor that doesn't want to do an episiotomy. And...sometimes they are just necessary for the baby's sake. But warm compresses, warm oil and stretching out the area all help. I have only seen midwives do this, doctors do not sit with their patients. Nurses do.

You should not insist on its avoidance as this procedure helps protects a mother from tears which can be very bad and problematic throughout the rest of the life. An episiotomy can be performed painlessly and can be repaired perfectly well, so why worry.

Consult a midwife, see what they cost. During the birthing process, they gently warm the area and massage it with oils to make it as supple as possible. Also, lying on one's back puts too much strain on the perineum, so try giving birth in a birthing chair instead.

Talk with your dr. before the delivery day... Tell him/her your wishes to not have an episiotomy... Make sure you tell the nurses at the hospital your wishes easly on in your labor and make sure they note it in your chart..

Perineum massage as discribed in the link can help prepare the perineum (skin ect between the vagina and anus) for the streatching it will be doing...

definitely don't try to push thebaby out until you are 100% efaced and at least 10cm dilated. but its better to tear than get an epesiotomy, since it will heal faster.
