These aren't exactly rhymes, but, here you go...
The Nightmare Before Surgery

“Twas the night before surgery, and all through the ward,
The nurses looked tired, so listless and bored.
I in my nightgown, strapped to my bed,
My upcoming procedure filled me with dread.
The pulse ox measured a strong 99,
While the EKG showed that my heart was still fine.
But the pain in my head would not go away,
As I worried about my surgery next day.
Then, up at the monitor it began to beep loud!
Throwing my mood into a dark cloud.
The nurses rushed in to see what was the matter,
Someone knocked over a tray which clanged and it clattered.
The team then rushed me to the Operating Room,
My doctor's face was worried with gloom.
They gave me the gas before I could protest,
Which put me right out like an unwanted house guest!
I awoke in recovery, still groggy and sore,
All I wanted was some water, and nothing more.
They gave me some ice chips to moisten my throat,
It wasn't that much, but it was more than I'd hoped.
After three hours I was wheeled back to my room,
By a huge patient transporter, a monsterous goon.
He picked me up gently, to my thankful surprise,
And placed me in bed as if I were pint-sized.
The surgery went well, there had been nothing to fear,
The staff were all great, thoughtful and dear.
But I feel like I've been through a grinding grist mill,
I know the worst part is coming – THE HOSPITAL BILL!"

Well, here are a few ideas:
Physical health/emotional wealth
Good nutrition/that's my mission/my ambition
Good nutrition/health comes to fruition
Exercise/can't believe my eyes
Good hygiene/let's keep it clean
Eat lots a green/keep the body lean
Keeping it lean/eat protein
Don't be a brat/stay away from fat
Come to a halt/don't use that salt
Be smart/protect your heart
Don't cheat/cut the sweet
Take the healthy route/eat some fruit
Skip the Chips and dip/eat healthy, get a grip
lean, mean/healthy machine
eight hours of sleep/good health you'll reap
