Any childrens' hospital, but you would have to be referred by your doctor. Most school boards have psychologists on staff who would do it if you think your child will have problems in a regular class.

One of the easiest ways of having a child's IQ tested is to contact the psychology department of a University which grants Doctoral degrees in Clinical Psychology. These will be free, and will give you an accurate recording of the IQ. However, graduate psych students are not licensed, and as such the information cannot be used for school or legal purposes. If you need this for such purposes, than contact a licensed clinical psychologist in your area. Any child psychologist can do this, and actually any psychologist at all is able. The test is called the "Wechsler Intelligence Scale for Children", or the WISC-IV.
