You want a fishing knot tool.
Here's one at LL Bean for $7
http://www2.llbean.com/product/20761.htm
Also try looking at this one from Cabelas
http://www.cabelas.com/cabelas/en/templates/links/link.jsp;jsessionid=SUTONIPSK5TD4CWQNWSCCNIK0BW0KIWE?id=0037403319389a&type=product&cmCat=perf&rid=0180101070502&xpid=k17401&cm_ven=Performics&cm_cat=Affiliate%2Dclick&cm_pla=Nextag&cm_ite=DDI%20Link&afsrc=1&_requestid=68051
And look at this one too
http://saltfishing.about.com/gi/dynamic/offsite.htm?zi=1/XJ&sdn=saltfishing&zu=http%3A%2F%2Fwww.knottying.com%2F
All of the listed knot tying tools are less than 15 bucks. I've never used one but I think its cheap enough to try out. Hope it works for you!

There are a lot of knot-tying tools out there, but I am not sure which are better (or even usable) for someone with arthritis.
I am near needing one myself, I think.
