Generally what seems to happen is one company actually owns the phone lines and then smaller companies pay to lease those lines. That why if you owe money to your 'big' phone company, you can call places like CallNet or 1800for a phone and still get phone service in your house through your existing line.

In the UK you can have several types of phone service. You can pay both your line rental and your calls to one provider, or you can pay your line rental to one, and route all your calls through another provider. This is called CPS (Carrier Pre-select) You can have CPS to automatically route all or certain calls (like international) through your call provider. Or you can enter a code before each call to do the same thing.
