I'm filling up the PIO for my USC husband and I have a couple of questions.?

Question

In Question #7 the form asks for a 'verifiable address in India'? What address do I put in there? We currently live in the States (I am a Green card holder). Should I put my parents' address in India (it's the same address on my Indian passport?). In Question #14 I assume I put my passport details in. What is required for proof of residence?? Is this proof of residence for an Indian residence or for our residence in the States?

Also, I'm confused as to whether we have to send in my husband's original US passport or a copy?

Don't know what a PIO form is, but if it's an Indian thing, call the Indian consulate and ask.
