that happened to me once, i just rang my mobile service provider and they told me what to do. Sometimes the codes are as easy as: 0000

The PUK code is a long number, when you buy your phone they fit a small card (a sim card) into the back of the phone, this small card was punched out of a bigger one the size of a credit card, find this card and you find your PUK number.

but you have got to call your service operator, who will be able to tell you what PUK to enter to unblock your phone.

if you haven't set your password for locking your phone normally your phone company sets the last 4 digits of your cell number as the default. Try that.
