I'd say it's just a typical diaper rash. Your neighbor calls it a rubber pants rash because it is caused by the rubber pants holding in the moisture. But in the end (no pun intended) it's just diaper rash.
Look around in the baby section at the store. When my son was little, I found a cover that was more breathable than the rubber pants yet still kept the diaper from leaking.

I'm a Mr. Mom, I've always used cloth diapers and plastic waterproof pants on my kids when they were in diapers.
The only time I've seen the pants cause any kind of a rash it has been where the elastic touches the skin around the waist and leg openings.
This was always caused by the elastic getting soaked with urine and then being held tight against the skin because of the nature of what the elastic band is supposed to do.
Once the pants were washed and hung out to dry then they no longer would be a problem unless the elastic would get wet again.
My kids are all grown now and have kids of their own and they have the same issues with the pants, only where the elastic touches the skin.
They have used both cloth and disposable diapers and with the cloth diapers you have much less problems with diaper rash, even with the protective pants.
With disposables the waterproof covering is still plastic with a thin paper cover over it for that "cloth" like feeling. This actually puts the plastic in contact with the baby's skin more so than you do with plastic or rubber pants over a cloth diaper.

I used cloth diapers with my first child and rubber pants too. He never had any more diaper rash than any other kid. Rubber pants do not cause the diaper rash. The elastic on them might cause a rash, but diaper rash, I do not think so. Switching detergents can cause the rash also. I have never ever heard of rubber pants rash.

Without being able to see the rash, it's impossible to say.
Here are some ideas:
It might be a yeast rash caused by too much moisture/not enough air flow. This rash would not clear up quickly with use of diaper cream, it would need a special treatment.
It might just be heat rash.
It might be a reaction to something in his diet or in the laundry detergent.
If it is a yeat rash, you can put some "live and active culture" yogurt on it until you get a chance to see the doctor. The "good bacteria" in yogurt fight yeast infection.
If it is a heat rash, leaving him coverless sometimes to get airflow will help. This will also help treat/prevent yeast rash.
If it is due to detergent, a good rinse and using a non-allergenic detergent will help.
Have you considered wool covers? They are very breathable, but they do not leak! It's absolutely incredible. They are expensive to buy, but if you sew even a tiny bit you can make two from an old wool sweater at the thrift store!

I used cloth on my two youngest children. I have never heard of a rubber pants rash, it sounds silly to me. If he wasn't having allergies from the rubber before why would he start now. It is more likely something he ate. Or perhaps he sat in pee longer than usual. Or...did you change the way you wash them?
There is a great site with natural type moms and lots of them use cloth. Check out mothering.com and click on discussions. There is an entire section geared towards cloth. You will learn more than you ever though possible about cloth diapering.

I cloth diapered both my kids and for over 4 years we only had one diaper rash when my daughter had the flu (diarrhoea). I have never heard of rubber pants rash.

My mom used cloth diapers and rubber pants on us and we never had diaper rash unless we were teething. Is it possible that this could be happening? Did you change the brand of laundry soap that you wash them in? Unless the rash is where the elastic hits, I doubt it is from the pants.

Im not sure if it still is or not??? But rubber is made from a tree sap that some people re highly allergic to. It is an old expression and I don't know if they still use tree sap to make it or not so I can't say that's what it is? It could just be that plastic pants don't breathe as well as cloth or fiber diapers.
Slop some zinc cream on the baby's butt and take it to the peditrician on monday. Lose the plastic pants... They don't allow air flow.
