You need to do 200 minutes of cardio a week. Remember fat does not turn into muscle, so you need to lose your fat while putting on muscle. The 45 minute thing is not true, but don't over 60 minutes at a time.
Do 5-10 minutes of cardio to warm up, then do your weight training. Do 30 to 60 minutes more of cardio, then go home.
The best time to eat is just after your workout within an hour when your metabolism is raging. For your muscles, incorporate some isolated soy protein or whey protein. Whey is more potent but the soy tastes better!
Don't look at your scale as the be all and end all, but get a measuring tape to check your progress, and that will show more tangible results.
Good luck and stick at it.

A sure way is what everyone else says - healthy, low calorie foods and consistent exercise. However, this is a really quick way to drop a few pounds. The longer you stay on it, the more weight you will lose, but it will probably come back. Anyway here it is:
Multivitamin
Breakfast - Slimfast
Lunch - Slimfast
Dinner - Lean Cuisine
Lots of water. Walk or walk/run at least 30 minutes every day.
This is getting into starvation, anorexia stuff, by the way, so be careful. This is a diet, not a lifestyle, and is not neccessarily healthy (it has a good amount of sodium, and doesn't have a complete variety of nutrients).
Good luck!!

A sensible diet and a good work out routine used at least 3 days a week. Make it something you enjoy, I do mountain biking on top of going to the gym and doing light cardio followed by a weight training plan and finish with a 30 minute cycle. If you make it something you enjoy doing you will have more luck. I used to run in the Army and didn't mind it but I tried to do it again to lose weight and hated it so it never worked for me as I made excuses not to go for a run!

Make sure you eat a balanced diet that includes less calories than those you burn off during regular exercise. Weight Watchers is an excellent place to go to learn how to do this properly and healthfully.
