The cotton is for shipping. The bottles have the proper pill count, but that does make the bottles full. So they put in the cotton, so they don't rattle around and break.
Once you open the bottle, you may discard the cotton. But you should put the cap back on for two reasons. First, to prevent the pills from spilling all over if it gets knocked over. Second, so the pills don't get damp when the weather is damp.
There is nothing wrong with getting an Easy Open bottle from your local drugstore and MOVING your pills into that. In this case, the top will come of easily, so you don't mind keeping the cap on.
