Formula or breast milk, with a bottle, about 10 diapers, box of wipes, desitin, sanitary hand cleaner, infant tylenol, snacks (if older), a juice (if older), extra onesie, extra outfit, socks.
It all depends on the age and what your child goes through throughout the day. Only you can be the judge of that.

Is it a bag to pack for your baby to bring him/her home? Make sure you have extra clothes (wise to buy at least one preemie outfit), cap, booties, plenty of diapers and wipes, the baby book for you to fill out, and anything else you would like your little one to have. Also make sure before you put the carseat in your vehicle that the straps are adjusted for newborns...since the doctors and nurses at the hospital may not help you to adjust the straps or show you how to use the carseat.
If it's just a general diaper bag for regular outings, always keep plenty of diapers and wipes stocked in the bag. Include the necessary treatments for their bottom (powder or cream), and keep a few extra bibs in your bag. My 15 month old son has at least 2 toys that are in easy reach for trips, and those are packed in the bag as well. I personally keep an extra immunization record and emergency contact numbers (including the pediatrician) in with his things too, just in case he spends the night somewhere and there is an emergency.\

All the normal things you would need at home when you change your baby's diaper.
Obviously some spare diapers to start with, lotion or powder if you use that for your baby. A changing pad to lay your baby on when you have to change a diaper. Wet wipes for cleaning your baby in case you aren't near a restroom or some other source of soap and water to clean the baby with.
You should have spare diaper pins if you are using cloth diapers and maybe even carry some if you don't, for other things that may need pinned. You may need to pin the baby's pacifier to their clothing or something like that, or the snaps on his/her pants or something may not work any more and you need to pin things closed.
A spare pacifier, several bottles (formula, juice, water) for drinking and feeding the baby. If the baby is older then you need to carry baby food with you as well as burp cloths.
Possibly a light weight blanket, and for sure at least one change of clothing. You need to be prepared for most anything. You may even want to carry a spare blouse or something for yourself in case the baby spits up on you or wets/messes on your clothing.
Mostly just think of what you use at home for the baby and carry smaller sizes in the diaper bag. It should be sort of a portable nursery.

6-8 diapers, baby wipes, extra clothes,receiving blankets, large cotten blanket for comfort, favorite toy, some bottles, baby formula or breast milk, 2- burping rags, couple of bibs, hand sanitizer, baby powder (if you beleive in using it on a newborn), diaper rash cream, nail clippers, wash clothes, baby shampoo, baby lotion, combs, infant tylenol, and rubbing alchohol and q-tips! I know this may seem like alot but its the perfect kit for the diaper bag.

It depends on how long you will be gone from home. I usually pack the following for about a 5 hour time.
Three changes of clothes
five to ten diapers
a small box of wipes
a plastic walmart bag - for dirties
diaper cream
a pacifer if your baby takes one
bottles with formula if your baby takes formula
a bib if baby is bottle fed
nasal bulb if baby has drainage
you a change of shirt if you nurse

Diapers, wipes, a bottle, depending of the age of the baby, a nasal aspirator, depending on where your going, lotion and baby soap. There are a lot of things that are going to depend on where you plan to go, how long you'll be gone, etc.
