It means that they have failed to make any difference between the enemy that they are fighting and the civilians that happen to live there. they will kill one just as easily as the other.

It means the nation is killing or imprisoning civilians. Ask someone from London or Dresden, Germany who was around during WWII...they'll tell you what it really means.
