And it looks like it has turned the caps lock on as well...
What you do is you right click on Start, select Properties, click Customize, then click Clear List.

Go to microsft.com and search for "power toys". You will find a free program called Tweakui. Download ans install it. It give you a simple menu for clear the run, recent documents and dozens of other list like that. Plus it allows you to tweak settings on a lot of other Windows items that you normally can not do. It should required on every PC. :-)

For Windows NT4 try this, you will have to have Admin access. I think this comes in Group Policy Settings. Which means you will have to play around with some registry settings
Policy
Setting Remove Run menu from Start Menu
Description
When this policy is enabled, Windows removes Run from the Start Menu and disables users from launching the Run dialog via pressing the Windows Key + R.
Application Action
If your application has a "run" function that allows a user to start a program by typing in its name and path in a dialog, then your application must disable that functionality when this policy is enabled.
Registry Information
Key: HKCU\Software\Microsoft\Windows\CurrentVersion\Policies\Explorer
Value Name: NoRun

Policy Setting
Do not keep history of recently open documents
Description
When this policy is enabled, the system does not save shortcuts to most recently used (MRU) documents in the start menu.
Application Action
When this policy is enabled, applications must not display any MRU lists (for example, in file menu and save/open dialogs).
Registry Information
Key: HKCU\Software\Microsoft\Windows\CurrentVersion\Policies\Explorer
Value name: NoRecentDocsHistory
____________________________________________
Policy Setting
Hide the dropdown list of recent files
Description
When this policy is enabled, Windows removes the MRU list from the common dialog.
Application Action
When this policy is set, applications that provide their own file/open dialogs must not display an MRU list in these dialogs. Applications that use the Windows common dialog API will automatically comply with this policy.
Registry Information
Key: HKCU\ Software\Microsoft\Windows\CurrentVersion\Policies\Comdlg32
Value Name: NoFileMru

If you are Using Windows XP with Classic menu Start Option, then, Click Start - Settings - Task bar and Start Menu. This will open a new window, Choose Start Menu Tab on top, Click Customize, Then Click "Clear" button, Click OK, Click OK.
If you are using Windows XP with XP Start menu then you will need to access this option through the Control Panel, Click Start, Click Control Panel, Double Click Task Bar and Start Menu Icon, click on the Start Menu Tab, then click Customize, Then on Advance Tab and make sure that the last option List my most recently opened Documents is not checked, if it is either Uncheck it or Clear the list, if u uncheck it then it will not display in this setting.
If you are allowed to download and install software in you system then you can use Windows & INternet washer pro from www.ewasher.net, this supports NT4
