What was the process you went through to get to America? I hear there is a interveiw in Toronto. What was that like?
What about the medical? Is the medical in Toronto? Montreal? Could you use your own doctor?

Otherwise, you are going to want to be able to prove anything you say. Document, document document. Have pictures, phone records, emails... be upfront and honest and you should be okay.
As for the doctor, I think there are only certain doctors who are "certified" to do the examination needed. I bet there are some in every major city - the guy I know did his in Ottawa. Read any and all instructions very carefully and follow them to the letter.
