Check out Banana Nana.com they have a ton of ideas and great gifts for Grandmas...Camp Grandma namebrand has adorable gifts at reasonable prices as well. Last year my daughter was 3 and got her Gram stuff from there the, Camp Grandma Calendar/ coffee mug/ tshirt/ and photo album...very very cute!
Have fun with it...also cute handmade gifts like take her picture and put it on one of those cute precut foamies and have her decorate it...put a magnet on the back to stick on the fridge also a fav of my mom in law. :)

I think anything that is home-made.. Well it depends on when grandma's b-day is.. but one gift I gave my mom when I was little that she really liked was a heart cookie cut out with clay. With some type of pencil or tool write on it.. I love grandma.. or I love you.. and you can use some kind of color glaze on it as well.. and then poke a hole in the top and make it into a necklace with a cord or string.. Also I made a christmas ornament once w/ my picture on it that my grandma really liked it was like a snowflake made out of popsicle sticks.. or you could do a clay cookie cutter ornament as well.. basically anything home-made and she'll probably love it.. especially at that age :-D (they're not 3 forever and they grow up soo fast.. )

When I was about that age, my parents gave me a crafting kit that would make little leather pouches that were for key chains. They were already cut and had the holes in them,(they also already had a snap afixed for closure.) all I did was put the lacing in to hold it together.
Granted the next part was my own idea. I had about 13 cents to my name, I put it in the pouch and gave that to my grandma. When she died she had the same pouch, with the original coins still in it, that she had instructions to have given back to me. I would say that just about anything a child that young makes for grandma will be a cherished treasure. I know I still have that pouch, with the orignial gift of coins intact..

Make grandma a t-shirt or a handbag with the three year olds hand prints with paint, grandma would love that, it would be something she could hold dear.
