I was an EMT for 8 years. The service I ran with asked you to sign a waiver indicating that you did not want our medical assistance, and that you would not hold us liable for injuries you may discover latter on.
If you are injured, even just a little, from a legal standpoint, go your self and get looked at just to be safe.
The human body doesn't always feel pain in situations like these, and injuries start to hurt 2 to 3 hours latter.

You can sign an "AMA", against medical advice if you were deemed alert and oriented, but if you fall within the criteria that your injuries impair judgement and you are not able to make a rational decision regarding your own health and well being, the medics may call upon the police officers to issue a mandatory transport under the guise of "danger to self". This is rare. At best they will advise you of the possible complications of refusal based on symptoms and injuries and you can refuse.

If you refused to get on, they're not going to point a gun to your head and make you. It's your choice! If you were seriously injured, then I suggest you do cooperate and get on the stretcher to be taken to the hopital to be examed and treated for.

The paramedics will suggest that you go get checked out in the hospital. However it is up to you whether to heed their advice or not.

If you are in command of your faculties, no they can't. It would be the same as a doctor treating you without your permission. However, if you are unconcious or incoherent they are duty-bound to treat you. Their duty is to save lives.
