tell her that u wanna go over and deliver it once your there keep the shirt in the car and ask her if she wants to go to the mall,dinner,miniput or sumtin like that and thats all that i can say

Just find a way to be with her and tell her that she forgot her hoodie at your house..when you give it to her ask her out.

be polite about it, just mention it, and suggest you meet up to go for a coffe or something so you can give it back, don't ask for a "full" date at first, she needs to get used to you and feel comfortable, then maybe you can ask her out on a full date...
